In a nutshell

  • You may fill out the Japan visa application form using a computer. You may also submit a handwritten form using black ink (Avoid erasures or corrections).
  • Type in block or UPPERCASE letters.
  • Make sure to provide the area code when providing contact numbers.
  • Don’t leave any fields or items blank. Type in ‘NA’ for questions that are irrelevant to you.
  • Print your filled-out Japan visa application form on A4-size papers. Avoid using any other size like legal or short.
  • You may also submit a handwritten form provided that you use black ink. Avoid erasures or corrections.
  • Attach your recent 2×2 photo using paste or glue. Do not staple it to the form.

Where to Download the Japan Visa Application Form

Download the Japan visa application form by clicking below. After typing in your answers, print the two pages on A4-size papers. (You are not supposed to staple them together nor use any other paper sizes). Don’t forget to attach your photo and sign on the last page.

Step-by-Step Guide: How to Fill Out the Japan Visa Application Form

Below is a visual guide on how to fill out the visa application form. For emphasis, I used fonts in red, but remember to use black text for your responses (or black ink, if handwritten!).

1. Application Photo

The Embassy of Japan in the Philippines provides the following image guidelines for strict compliance:

  • Paste your photo to the designated section. It will not be accepted if it is stapled.
  • Your photo must meet the size requirement of 4.5 x 4.5 cm. A photo that has a different size will not be accepted.

Furthermore, your picture must be taken within the last 6 months prior to submission, must show your neutral facial expression (don’t smile!), must show your uncovered face (no caps and sunglasses!) must not be bordered, and must have a white background.

To easily meet all these criteria, I suggest visiting a nearby photo studio and avail their Japan visa package. Once you have your application photo, write your name and birthdate on the back side of the photo.

2. Basic Information

The first few parts of the Japan visa application requires basic information about yourself:

Surname

Write your surname as shown in the passport. (Remember to use block letters, as in ALL CAPS!)

Given and Middle Names

Provide your given name/s and middle name on the same line.

Other Names

If you have changed your name, write the previous. Otherwise, type N/A.

Date of Birth

Use the format DD/MM/YYYY, e.g. 14/02/2000 for February 14, 2000

Place of Birth

Follow the prescribed format when keying in your place of birth: City, State or Province, Country. 

Sex & Marital Status

Put an X or a cross mark on the applicable boxes.

Nationality or Citizenship

Write your nationality, e.g. Filipino. 

Former and/or other Nationalities or Citizenships

If you are (or have been) a dual or multiple citizen, type in the information. Otherwise, use N/A like I did.

To provide an example, if you are a Filipino national who’s a dual citizen of the Philippines and the United States, you can write PHILIPPINES on Nationality or Citizenship and UNITED STATES OF AMERICA on the Former and/or other Nationalities or Citizenships.

ID No. issued to you by your government

You can use the number found in any of your valid Philippine identification cards, such as driver’s license, SSS, GSIS, UMID, Senior Citizen’s ID, Voter’s ID, or even your passport.

3. Passport Information

To answer the next six items, refer to your valid passport:

Passport Type

Tick Ordinary like I did, unless you are a government official or employee who holds a Diplomatic or Official passport.

Passport No.

If you have the latest passport with the 10-year validity, the format is P1234567H (or 7 digits in between 2 letters).

Place of Issue & Issuing Authority

Type in the Place of Issue, e.g. DFA NCR WEST or DFA MANILA. For Filipinos, the issuing authority is DFA.

Date of Issue & Date of Expiry

Again, use the format DD/MM/YYYY for both items.

4. Travel Details

Purpose of Visit to Japan

Write TOURISM if you plan to travel for sightseeing or leisure. If your main or sole purpose is to see your friends or family, write VISIT FRIEND/FAMILY.

Intended Length of Stay in Japan

Write the number of days. (If still unsure, you can indicate a tentative length of stay in this portion.) 

Date of Arrival in Japan

Indicate the date when you plan to enter Japan.

Port of Entry into Japan

If arriving by flight, common responses to this part of the Japan visa application form are Narita, Kansai, Chubu, Haneda, Osaka, Chitose, Fukuoka, and Naha. You can find the list of airports here.

Name of Ship or Airline

You may provide a tentative response, i.e. Philippine Airlines, Cebu Pacific, ANA, and others.

Names and Addresses of Hotels or Persons with whom Applicant Intends to Stay

For tourists, write down the name of the hotel or hostel that you plan to book for your accommodation. Include their address and telephone number. (You must include the area code for the contact number!)

Dates and Duration of Previous Stays in Japan

Write N/A if you have not traveled to Japan before.

If you have, provide the dates and duration of your trip/s, e.g. FEBRUARY 4-17, 2018; 14 DAYS or DECEMBER 30, 2019-JANUARY 5, 2020; 7 DAYS.

If your response to this portion of the Japan visa application form no longer fits the provided line, you may use a separate sheet and indicate something like ‘Continued on separate sheet.’

5. Personal Details

Your Current Residential Address,   Telephone No., and Mobile No.

Provide your present place of residence. If you have another address, you may use a separate sheet to provide the information.

Type in your telephone (with area code!) and mobile numbers. If you have no landline at home like I do, you can use N/A. 

Current Profession or Occupation and Position

Key in the response that best describes your present occupation. For example, if you have a business, you may put BUSINESS OWNER. Those who are employed may write their position, e.g. ACCOUNT SPECIALIST or COPYWRITER. 

Name and Address of Employer

Provide the name, address, and telephone number (with area code!) of your employer. 

Partner’s Profession / Occupation (or that of Parents, if Applicant is a Minor)

This part right here is already found on the second page of the Japan visa application form (Yep, you’re halfway there!).

If you are single and of legal age (18+ years old), type in N/A. Otherwise, provide your partner’s or your parents’ occupation.

6. Details of Your Guarantor or Inviter

The last few sections of the Japan visa application form asks details of your guarantor or inviter (can be one person!) from Japan. If no one invited you to travel and if no one would sponsor your trip expenses (partial or full), use N/A.

If Applying for a Japan Visa With a Guarantor and/or Inviter

If you have someone from Japan who will act as your guarantor and/or inviter, provide their details and your relationship with them on the applicable portions of the form, e.g. SISTER-IN-LAW. If the guarantor and inviter is the same person, type in ‘SAME AS ABOVE’.

7. Personal Background Check

The second to the last part of the Japan visa application form mostly seeks to know your criminal history, if any. Read each question and answer truthfully. If you answered ‘Yes’ in any part, provide relevant details inside the box provided. Should you need more space, you may opt to use an extra sheet.

Remember that, as a general rule, providing false information to visa applications can lead to denial of issuance of visa, among other consequences.

8. Date & Signature

Date of Application

Follow the format of DD/MM/YY for consistency, i.e. 25/02/23.

Signature of Applicant

Once you’ve printed out the Japan visa application form, you can sign using a pen with black ink. (By providing your signature, you declare that you’ve supplied only true and correct information in your application. You also agree to the other terms set forth by the Embassy of Japan in the Philippines.)

A parent or a legal guardian may sign this part if the Japan visa applicant is a minor. The relationship must be noted below the signature, e.g. SIGNED BY MOTHER
